Call 1-888-321-5880 and follow the prompts to get an application mailed to you for Lifeline Service.
If you applied from California or Texas, your paper application will be processed automatically for Assurance Wireless Lifeline. If your paper application is for a state other than California or Texas, once your paper application is approved, contact 1-888-321-5880 to receive a link to begin your Assurance Wireless service application.
