Provide Your Information

Let's begin your online application. A few simple questions and you're on your way to being connected on the T-Mobile Network. This process should only take 7-10 minutes. The National Verifier Process

During your Assurance Wireless application, you will be redirected to the National Verifier website, which is Lifeline's centralized application system. It validates your identity and address, and determines your eligibility for Lifeline.

California PUC

After you submit your Assurance Wireless application, we'll securely send your information to the California Public Utility Commission (PUC). The PUC will carefully review your application details to verify your information and confirm your eligibility.

Texas PUC

After you submit your Assurance Wireless application, we'll securely send your information to the Texas Public Utility Commission (PUC). The PUC will carefully review your application details to verify your information and confirm your eligibility.

Providing Proof of Eligibility

If you're applying through a government program or based on your income, the National Verifier might automatically check your details using databases for programs like Medicaid, Housing Assistance (FHAP) and Veterans Benefits (VA). If they can't find your information, you'll need to upload proof of program enrollment or income documents.
