It's easy. Just follow these steps:
- Fill out the California LifeLine Request Sheet making sure to include your mailing address and create an Account PIN and Secret Answer. Mail it back to Assurance Wireless.
- Watch your mailbox for a pink envelope — the official California LifeLine Application Form from the California LifeLine Administrator.
- Fill out, sign and send in the official California LifeLine Application Form to the California LifeLine Administrator.
- Once you receive your FREE phone from Assurance Wireless, follow the instructions to activate it immediately so you can get LifeLine approval from the California LifeLine Administrator and start calling, texting and surfing the web.
- You will then receive an Approval letter from the California LifeLine Administrator.

Good news. If you already receive California LifeLine discounts you can still switch to the California FREEdom® Plan with Data! And keep your current phone number.
