If you are a customer of Assurance Wireless, a Lifeline Assistance program, we are happy to serve you. Each year, Federal and/or State regulatory authorities require that customers certify their eligibility for the Lifeline Assistance program within 60 days of their annual enrollment.

By checking the checkbox and clicking the “Submit” button below, I hereby certify, under penalty of perjury, to the following five statements:

I acknowledge that providing false or fraudulent information in order to receive Lifeline Assistance is punishable by law.
|

|
2)
|
I currently participate in one or more of the Lifeline-qualifying public assistance programs or qualify for Lifeline based on household income.
|
|
|
3)
|
I understand that Lifeline is only available for one phone line per household, whether landline or wireless. I am the head of household and will only receive Lifeline from Assurance Wireless.

4)
|
I am personally eligible for Lifeline and my Assurance Wireless service has not been transferred to me by any third party, and I will not transfer my service.

5)
|
I will notify Assurance Wireless immediately if I no longer qualify for Lifeline, or if I have a question as to whether I would still qualify.
